Match Cards

Following the AGM in July 2011 we have reverted back to having a single match result card. The new 2011-12 match cards are in Adobe Reader format therefore enabling managers to complete their own cards prior to arrival.
The match result cards MUST be checked against the ID cards once on site!
The match card can be used for all GJFL League and Cup matches. The card is in Adobe Reader format which is free software which can be downloaded from www.adobe.com/reader

An extra field “Team Name” has been added in order for us to distinguish which team the player information relates to when we receive it.

All fields on the form must be completed before the Match Result Card is submitted to the Divisional Secretary.

The form is designed to be pre filled with as much information as possible before each match and then printed.
Alternatively you can printed blank copies and fill the form in manually.

At the end of each match the team manager must complete their own card with the match result, referee scores and indicate in the box provided if any player did not participate in the game.

Both managers should sign their own card and their opposing teams card before the cards are swapped.

The match result card can then be scanned and emailed to your Divisional Secretary or posted. Please contact your Divisional Secretary for information on how to send match cards to them.

Please note that all information must be completed in full as fines are automatically applied if information is missing from the card. Please note each manager will be responsible for sending in his OPPONENTS match card and a fine will be issued for non receipt.

Don’t forget, these must be e-mailed, posted, faxed or whatever way your Divisional Secretary has requested within FOUR days of the match.
Please make sure you have no duplicate players and that every player had an ID card and that IT WAS CHECKED!
